Job summary

Premier Nursing and Rehab Center of Far Rockaway seeks an experienced Licensed Practical Nurse (LPN) to provide direct nursing care under the supervision of an RN or Nursing Supervisor. You will administer medications, monitor residents, and document care in the EMR.

You will supervise CNAs, assist with admissions and discharges, maintain infection control, and communicate with residents and the interdisciplinary care team. An active NY LPN license and CPR/BLS certification are required.

Qualifications

• Current New York State LPN license.
• Graduate of an accredited Practical Nursing Program.
• Current CPR/BLS certification.
• Strong communication, organizational, and interpersonal skills.
• Ability to work effectively in a fast-paced skilled nursing environment.

Responsibilities

• Provide direct nursing care per individualized care plans.
• Administer prescribed medications, injections, treatments, and wound care.
• Monitor residents for condition changes and report findings to RN/physician.
• Complete accurate documentation in the EMR.
• Assist with admissions, transfers, and discharges.
• Supervise CNAs and delegate care assignments.
• Ensure resident safety and infection prevention standards.
• Respond to resident needs, emergencies, and changes in condition.
• Communicate with residents, families, physicians, and care team.
• Participate in care planning meetings and quality initiatives.
• Maintain compliance with NYSDOH, CMS, and facility policies.
