Job summary

Anniston Health and Rehab in Anniston, AL is seeking a Licensed Practical Nurse (LPN) for a full-time, hourly position with shift differential. The role provides direct resident care in our new building and requires a current Alabama LPN license and CPR certification within 30 days.

You will work 7a-7p or 7p-7a shifts, supervise Care Assistants, document care, and coordinate with physicians and families to ensure quality nursing care.

Qualifications

• Active LPN license in Alabama.
• At least 1 year of care facility experience preferred.
• CPR certification must be obtained within 30 days.
• Ability to perform nursing assessments and care planning.

Responsibilities

• Admit, transfer, and discharge residents as required and inform staff.
• Monitor residents' physical and emotional status.
• Direct Nursing/Resident Care Assistants and report concerns.
• Document according to policies and regulations.
• Chart nurse notes reflecting care provided and resident responses.
• Report discrepancies to the Resident Care Coordinator.
• Transcribe and verify physician orders.
• Administer medications per physician orders.
• Complete assessments on all assigned residents per shift.
• Communicate with physicians, residents, families and staff about condition.
• Provide direct care per care plan.
• Perform other duties including special projects.
