Job summary

TeemaGroup is seeking a Licensed Practical Nurse for a contracted full-time position with the State of Arizona. The role covers 6:30pm to 6:00am on 36–48 hours per week, handling medication administration, treatments, and on-unit patient care.

You will observe and report on patient status, participate in shift reports, and support interdisciplinary teamwork with a focus on geriatric nursing and QA processes.

Qualifications

• LPN license required and in good standing.
• Geriatric nursing theory and practice knowledge.
• Ability to perform wound care, medications and treatments as ordered.

Responsibilities

• Observe and report the effectiveness of medications and treatments.
• Ensure supplies and medications are available for on-unit use.
• Receive and relay resident information to the team and families.
• Participate in shift handoffs and QA processes.
• Communicate with residents, families and staff for coordinated care.
• Complete admission assessments and assist with care planning.
• Maintain documentation of diagnosis, observations, and progress.
• Contribute to QAPI program participation.

Schedule: 6:30pm to 6:00am (36-48 hours per week)

This job is a contracted job for the State of Arizona.

Administer medication and perform treatments on an assigned unit as ordered by the physician and within scope of license. (Medications may be in a variety of forms including oral, enteral, parenteral, suppository, etc. and treatments may include sterile and aseptic wound cleaning and dressing, postural drainage and percussion, tracheostomy, gastrostomy and jejunostomy care, and catheter care.)

• Observe and report the effectiveness of medications and treatments
• Ensure an adequate number of supplies and medication are available
• Receive and deliver a variety of information regarding the resident's medical, social, functional, and other needs
• Participate in the receiving and giving shift reports
• Communicate with residents, family, and staff as necessary to ensure the resident receives appropriate care from the interdisciplinary team
• Complete the initial admission nursing assessment tool on a newly admitted resident
• Assist with the development of the Minimum Data Set (MDS) assessments, ongoing quarterly reviews and care plans
• Maintain appropriate clinical and administrative documentation regarding diagnosis, observations, treatment, and summary of resident progress.
• Participate in our Quality Assurance and Performance Improvement (QAPI) program
