Ryders Health Management—Lord Chamberlain facility in Stratford, CT—seeks a full-time Licensed Practical Nurse (LPN) to deliver high-quality, resident-centered care. You will work under RN supervision, administer medications, monitor vital signs, assist with ADLs, and document care in medical records.

The role emphasizes compassionate patient care, teamwork, and adherence to infection control protocols, with a supportive environment and growth opportunities within our skilled nursing facilities.

Qualifications

• Valid, unrestricted LPN license in Connecticut.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR/BLS certification as required by facility policy.

Responsibilities

• Provide direct nursing care to residents under RN supervision or physician orders.
• Administer medications and treatments per orders and facility policy.
• Take and record vital signs and monitor changes in condition.
• Assist residents with ADLs including bathing, dressing, feeding, mobility and hygiene.
• Maintain accurate documentation of care and observations in medical records.
