Job summary

Silver Stream Rehabilitation and Nursing Center in Spring House, PA is seeking compassionate Licensed Practical Nurses (LPNs) to join our nursing team for day shifts (7:00 AM–7:00 PM). This role offers three 12-hour shifts per week, plus every other weekend, providing a stable schedule and work-life balance.

Responsibilities include administering medications, monitoring residents, documenting care, supervising CNAs, participating in care planning, and ensuring regulatory compliance.

Qualifications

• PA LPN license required.
• Long-term care experience preferred, but not required.
• Strong clinical, communication, and organizational skills.
• Ability to work effectively as part of a team.
• Commitment to providing exceptional resident-centered care.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor residents' conditions and report changes to the RN Supervisor and physicians.
• Provide direct nursing care in accordance with facility policies and standards of practice.
• Accurately document resident care and services provided.
• Supervise and support Certified Nursing Assistants (CNAs).
• Participate in care planning and interdisciplinary team meetings.
• Ensure compliance with all state, federal, and facility regulations.
