Job summary

Pillar Care Continuum in Wayne, NJ is hiring a Licensed Practical Nurse for the Day Program Services. This full-time role (Mon-Fri, 8am-4pm) includes medication administration, tube feedings, G-Tube care, hygiene, meals, and fostering community connections for adults with disabilities.

The LPN must have NJ licensure, at least 1 year of relevant experience, and strong health-regulation knowledge, with a valid driver’s license and TB screening.

Qualifications

• LPN license in New Jersey.
• Minimum of 1 year experience in an adult program, hospital, nursing home, clinic, or group home.
• Strong knowledge of health regulations, first aid, and emergency procedures.
• Excellent communication and interpersonal skills to work with consumers, guardians, and staff.
• Valid driver's license.
• Fingerprint examination and TB screening.

Responsibilities

• Participates in initial evaluations by reviewing medical records and assessing residents' health.
• Assists in developing individualized health plans and annual re-evaluations.
• Monitors daily health status, reports concerns, and notifies the Service Coordinator.
• Administers prescribed medications and provides first aid.
• Maintains medical records and writes necessary reports.
• Organizes medical/dental screenings and participates in clinics.
• Contributes to health, safety, and emergency procedures and staff health education.
