Job summary

Ft Washington Rehabilitation and Wellness Center seeks motivated LPNs to join our day and night shift team in a long-term care environment. The LPN will support residents through the nursing process, collaborate with physicians and multidisciplinary staff, and supervise assigned team members while promoting compassionate care.

We offer a supportive setting with opportunities for professional growth and a shuttle service to local bus stops for easier access to the Fort Washington location.

Qualifications

• At least one year experience in a Long Term Care environment.
• Must be a graduate of an approved college/school nursing program.
• Must possess and maintain a current state license as a professional LPN.

Responsibilities

• Identify resident care requirements by establishing rapport with residents and others.
• Monitor and chart health information and oversee staff, including medication administration.
• Provide emotional, psychological and spiritual support to residents and families.
• Promote resident independence by teaching self-care and medication understanding.
• Ensure quality of care by comparing health outcomes to goals and standards.
• Resolve resident problems using multidisciplinary team strategies.
• Maintain safe, clean environment by following procedures and regulations.
• Protect residents by adhering to infection-control policies and medication storage.
• Document resident care services in records.
• Maintain confidentiality of resident information.
• Continue professional knowledge through workshops and networking.
• Foster cooperative relationships among health care teams.
• Contribute to team effort by completing related tasks.
