Provide licensed practical nursing care to day and night facility residents.
Administer medications and treatments per physician orders and facility policy.
Assess residents’ health status and update medical records accurately.
Monitor and respond to changes in resident conditions promptly.
Document care and outcomes in electronic health records with accuracy.
Uphold safety, infection control, and compliance with facility policies.
Collaborate with interdisciplinary team to develop and adjust care plans.
Engage in continuous learning to advance clinical skills and certifications.
Demonstrate accountability for own performance and contribute to team targets.
Participate in daily rounds and handoffs to ensure continuity of care.
Support discharge planning and resident/family education.
Maintain PPE use and environmental cleanliness to ensure high visibility of work.
Valid LPN/Licensed Vocational Nurse license in state of practice.
Proven experience administering medications and treatments per orders.
Strong documentation skills in electronic health records with accuracy.
Ability to assess changes in residents’ health and respond promptly.
Experience in long‑term care or correctional facility settings preferred.
