Eastern Health Group is seeking an LPN to oversee the delivery of care to residents, providing direct nursing care under supervision of a registered nurse or physician. The role ensures high standards of care in line with state regulations and nursing home policies.

Qualifications

• Licensed Nurse with current state license.
• One year related experience preferred.
• Must have a current CPR certification.
• Excellent technical and documentation skills.
• Effective verbal and written English communication skills.
• Demonstrated proficiency with MS Word, Excel, PowerPoint and Outlook.
• Highest level of professionalism with confidentiality and teamwork.

Responsibilities

• Meet physical and sensory requirements and work in the described environment.
• Participate in process improvement to enhance workflow and patient experience.
• Collect information and assist with evaluations at admission and discharge.
• Ensure flow of work is completed by CNAs.
• Document services rendered to residents and chart essential information.
• Make daily rounds to monitor resident care and status.
• Implement care plans in line with physician orders.
• Follow established nursing policies and procedures.
• Notify residents or legal representatives of pending discharge 24 hours in advance.
• Chart discharge summaries including planning and referrals.
• Maintain good relationships with staff, residents, families, and vendors.
• Report disciplinary actions to Nursing leadership as needed.
