While executing patient care, exhibits knowledge of and maintains the patient’s right to diversity, privacy, and confidentiality.* As applicable to assigned area(s): Performs a standardized focused assessment or reassessment of patient at regular intervals appropriate for the condition of the patient under the direction of the RN and as ordered by the provider.* Administers medications, treatment, and provider-specific care to patients in compliance with nursing policies/procedures, LPN scope of practice, and as applicable to assigned area(s).* Communicates all appropriate and critical changes in the patient’s condition, status, and findings to the appropriate stakeholders in a timely manner and as defined by policies and procedures and/or provider orders as applicable to assigned area(s).* Documents all clinical findings and required regulatory mandates in the medical chart in compliance with policies and procedures or as defined by regulatory requirements as applicable to assigned area(s).* Contributes to maintaining an updated, individualized plan of care that includes; goals, interventions, medications and visit frequency orders as applicable to assigned area(s).* Maintains a thorough knowledge of infection control procedures and strictly adheres to policies and procedures.The above essential functions are representative of major duties of positions in this job classification.
