Job summary

Lifespace Communities seeks a Licensed Practical Nurse for GreenFields of Geneva in Geneva, Illinois. The role supports the care team and directly cares for residents on site.

The LPN will document conditions, administer medications, coordinate admissions and discharges, and collaborate with families and physicians to ensure individualized care plans. Hourly shifts (Morning/Afternoon) available, with PT and PRN options.

Qualifications

• LPN license required by state regulations (Illinois).
• Experience in long-term care or skilled nursing is preferred.
• Strong communication and teamwork with interdisciplinary staff.

Responsibilities

• Participate with the interdisciplinary team to review, plan, coordinate and evaluate resident's care.
• Document resident condition and nursing needs timely and accurately; report observations.
• Develop and monitor nursing assistant assignments and adjust per census and care level.
• Execute treatments and document responses to medications and treatments.
• Generate, prepare, administer, and chart all medications.
