Job summary

Eastern Healthcare Group is seeking a Licensed Practical Nurse (LPN) to oversee nursing care for residents in our Roanoke facilities. You will provide direct nursing care under the supervision of a registered nurse or physician, ensuring residents receive high-quality care in line with state regulations.

Key duties include charting, admissions and discharge support, collaborating with CNAs, implementing care plans, and maintaining infection control practices.

Qualifications

• Licensed Nurse with current state license.
• One year related experience preferred.
• Current/active CPR certification required.
• Excellent technical and documentation skills.
• Strong English communication skills (verbal and written).
• Ability to work independently and as part of a team.
• Maintain professionalism and confidentiality.
• Ability to manage multiple tasks with high accuracy.

Responsibilities

• Meet physical and sensory requirements and work in described environment.
• Participate in process improvement to enhance patient experience and workflow.
• Document services rendered to residents and write admission notes.
• Monitor resident care during daily rounds and follow physician orders.
• Implement care plans and follow nursing policies and procedures.
• Notify on discharge planning and complete discharge summaries.
