Ritechoice Family Services Inc in Philadelphia, PA is seeking a compassionate Licensed Practical Nurse (LPN) to join our growing healthcare team. You will provide direct patient care, monitor vital signs, administer medications, and participate in care planning to support individuals and families in the community.

You will collaborate with RNs and physicians, document treatments, educate patients and families, and help ensure high-quality, compliant care while maintaining a patient-centered

Qualifications

• Active LPN license in Pennsylvania.
• Graduate of an accredited LPN/LVN nursing program.
• Current CPR/BLS certification.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.

Responsibilities

• Provide direct patient care including monitoring vital signs, administering medications, and wound care.
• Assist in developing and implementing individualized patient care plans.
• Collaborate with RNs, physicians, and interdisciplinary care teams to ensure quality outcomes.
• Document patient assessments, treatments, and progress accurately in medical records.
• Educate patients and families on health management, medications, and discharge instructions.
• Respond promptly to patient needs and changes in condition, escalating concerns as appropriate.
• Maintain compliance with all state, federal, and organizational healthcare regulations.
