Job summary

Abbotsford Health Care Center is seeking an LPN to ensure patients receive needed care and adequate resources. The role supports outcome-focused nursing, collaborating with the health team to deliver high-quality care and meaningful patient outcomes.

The position emphasizes supervision of CNAs, accurate documentation, and adherence to care plans and physician orders in a supportive environment.

Qualifications

• Graduate from an accredited school of licensed vocational nursing or LPN with current state license.
• Current state license to practice as an LPN and CPR certification.
• ACLS certification preferred.

Responsibilities

• Supervises Certified Nurses Aides under direction of RN.
• Collects patient care data and monitors patient conditions and family dynamics.
• Contributes to development of the plan of care and discharge plan.
• Implements patient care utilizing care plan and physician orders.
• Documents the delivery of health care and nursing process per policy.
• Performs administration and documentation of medications and treatments.
