Qualifications

• Current LPN license in Massachusetts.
• Certificate in nursing (LPN) from an accredited program.

Responsibilities

• Deliver direct patient care, including administering medications, performing physical examinations, and assisting with treatments and therapies.
• Assess and document resident/patient condition, including vital signs and medical information.
• Develop and implement individualized care plans with the interdisciplinary care team.
• Provide emotional, psychological, and spiritual support to residents/patients and families.
• Monitor and report changes in resident/patient condition to team members and supervisors.
• Maintain accurate, detailed records and reports.
• Adhere to facility policies, procedures, and standards of care.
