Qualifications

• Active PA LPN license (unencumbered).
• Valid BLS/CPR certification required.
• Ability to manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment and communication skills.

Responsibilities

• Provide patient care per the written plan and physician's orders.
• Follow and implement individualized care plans by the Nurse Manager and team.
• Perform medication administration and treatments per the care plan and facility policies.
• Monitor residents and report changes promptly to supervising staff.
• Maintain accurate documentation per orders and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and families.
