Job summary

Centerstone is seeking a Licensed Practical Nurse to provide mental health and substance use services in community and office settings across Florida. You will assess clients, administer medications, monitor vitals and collaborate with psychiatrists, physicians, and social services to ensure quality care.

Qualifications

• High school diploma or GED required.
• Licensed in the State of Florida as a Practical Nurse.
• Must have reliable transportation and a valid FL driver’s license.

Responsibilities

• Conduct nursing assessments for assigned clients.
• Arrange required lab work and maintain medical records.
• Assist with medication pre-authorization and refills as needed.
• Attend interdisciplinary team meetings.
• Evaluate and document medication effects; report to psychiatrist.
