Job summary

Phoenix Home Care & Hospice in Independence, MO is hiring an LPN Clinical Manager to lead our Home Health team. This full-time role offers a Monday–Friday schedule with a strong emphasis on leadership, care coordination, and hands-on support for nurses and caregivers.

As the clinical leader, you will supervise visits, assist with care plans, ensure quality patient care and compliance, and liaise with physicians and families.

Qualifications

• Active Missouri or Compact LPN license.
• Home health experience preferred.
• Strong organization and communication skills.
