Job summary

UnityPoint Health's Community Memorial Hospital is seeking an LPN to join the Physician's Clinic. This full-time role offers 40 hours weekly with flexible 5x8 or 4x10-hour days, and a $5,000 sign-on bonus.

Qualifications

• LPN license held and current in Iowa.
• Graduate from an accredited School of Nursing.
• Current license or certificate in Iowa.
• High school diploma or equivalent.
• Understand clinic concepts and practices.
• Read, write, and speak English.

Responsibilities

• Provide direct patient care and clinical support in an outpatient clinic setting.
• Prepare patients for examinations, treatments, and minor procedures.
• Obtain and document vital signs, medical histories, and other relevant patient information.
• Partner with providers and clinic staff to support timely, coordinated patient care.
• Communicate with patients, families, and healthcare team members clearly and compassionately.
• Assist with telephone triage, patient questions, and follow-up regarding health concerns and progress.
• Document patient care, interventions, and clinical information accurately in the EHR.
• Provide coverage and support across Family Medicine, Behavioral Health, Wound Care, General Surgery, and other specialty services.
• Contribute to efficient daily clinic operations and a positive, team-oriented patient experience.
