Job summary

Signature HealthCARE is seeking a caring LPN/LVN to provide direct nursing care, oversee nursing assistants, and coordinate care for residents in a supportive, team-based environment.

You will work with the interdisciplinary team to develop and review care plans, ensure safe medication administration, and participate in quality assurance activities to maintain high standards of patient care.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
• Strong verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Supervise nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
