Job summary

Chelsey Park Health & Rehab is hiring a Licensed Practical Nurse to deliver high-quality clinical care to residents. Work closely with RNs, CNAs, and the interdisciplinary team to provide compassionate, person-centered care in a supportive setting.

You will administer medications, monitor conditions, document accurately, and mentor CNAs while upholding regulatory standards and quality care.

Qualifications

• Active LPN license in Georgia or eligible.
• Strong clinical judgment and critical thinking.
• Excellent communication and teamwork abilities.
• Reliability and commitment to excellence.
• Prior long-term care experience preferred but not required.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor residents' conditions and report changes to the care team.
• Provide direct nursing care in accordance with care plans.
• Supervise and support CNAs in delivering quality care.
• Maintain accurate and timely documentation.
• Ensure compliance with clinical policies and regulatory standards.
• Communicate effectively with residents, families, and team members.
