KAISER is seeking a Licensed Practical Nurse (LPN), CCS for Home Health in Portland, Oregon. The role involves direct and indirect patient care, tests and procedures under the supervision of a clinician and RN, within the LPN scope of practice.

Expect collaboration with a health care team and emphasis on patient education and safety. The position requires a valid Oregon LPN license (or compact) and prior home health or acute care experience.

Qualifications

• Minimum one year experience as a licensed practical nurse in home care or the acute care setting.
• Graduate of Accredited School of Nursing
• Current LPN license in Oregon or Compact License: LPN within 3 months of hire

Responsabilités

• Provide nursing services, treatments, diagnostic and preventative procedures within the LPN scope under RN supervision.
• Develop, maintains, and provides knowledge and skills related to nursing services.
• Provide direct patient care following established plan of care by registered nurse.
• Coordinate home health services and communicate changes to clinicians and supervisor.
• Advocate for patients and families to optimize care and resources.
• Assist with patient education and self-care instruction.
• Maintain charting in electronic medical records.
• Ensure safety and regulatory compliance in coordination with health care team.
