Job summary

Yukon-Kuskokwim Health Corporation is seeking a Licensed Practical Nurse (LPN) in Bethel for a full-time role. The LPN Case Manager will assess, plan, coordinate, monitor and evaluate services for patients and families across the care continuum.

Minimum 3–5 years nursing experience with Alaska LPN license in good standing, CPR/BLS, and proficiency in medical terminology. Generous PTO, holidays and extensive benefits are offered in a community-focused rural health system.

Qualifications

• Graduate from an LPN/LVN program.
• Current Alaska LPN license in good standing.
• 3–5 years nursing experience.
• CPR (BLS) certification required.

Responsibilities

• Foster quality, continuity and appropriate utilization of health care resources.
• Facilitate coordinated utilization of resources among patients, families, doctors, nurses, payers and home care services.
