Job summary

Legacy Senior Living is seeking a dedicated LPN to provide direct nursing care and mentor nursing assistants. You will coordinate care, supervise daily activities, and maintain respectful, person-centered interactions with residents and families.

The role emphasizes clinical excellence, documentation accuracy, and adherence to safety, sanitation, and Legacy protocols. State-licensed LPN with prior senior living or acute care experience is preferred.

Qualifications

• Current LPN license in good standing with the state board
• Experience in senior living, skilled nursing, or acute care preferred
• Strong medication-administration and documentation skills
• Natural leadership—able to guide, teach, and uplift nursing assistants
• Physically able to lift 50 lbs regularly and 100 lbs occasionally

Responsibilities

• Coordinate care on shift and guide nursing assistants
• Maintain accurate narcotic counts and report discrepancies
• Complete focused assessments and report changes to the DON
• Chart thoroughly and timely, reflecting care plans and resident responses
• Direct and mentor nursing assistants, ensuring clear daily assignments and proper handoffs
• Communicate shift priorities and resident updates at change-of-shift huddles
• Maintain a warm, respectful resident environment and protect confidentiality
• Ensure safety, sanitation, and infection control in rooms and common areas
