Job summary

Mohawk Valley Health System is seeking a Licensed Practical Nurse (LPN) to provide general nursing care within the Medical Group practices. The LPN will assist and advocate for patients and families, collaborate with physicians and the care team, and operate under the direction of licensed professionals.

Responsibilities include gathering measurements, documenting findings, preparing patients for exams, and participating in quality improvement while maintaining a safe, clean environment.

Qualifications

• Graduate of an accredited LPN nursing program.
• BSN or MSN from an accredited program preferred.
• Current New York State LPN licensure required.
• BLS certification within 6 weeks of hire.

Job Summary

The Licensed Practical Nurse (LPN) provides general nursing care to patients, families and caregivers in the Medical Group practices. The Licensed Practical Nurse (LPN) assists and advocates for the patient and families health in support of the patient centered approach and delivery model of care, and is an active care team member. Responsible for collaborating with physicians/ providers , registered professional nurses and multidisciplinary care team members. A Licensed Practical Nurse (LPN) can perform tasks and responsibilities within the framework of case finding, health coaching, and supportive care under the direction of the registered professional nurse or licensed physician or licensed health care provider. Nursing shall perform in accordance with the standards outlined for Licensed Practical Nurse (LPN) in the NYS Education Law, NYS Office of the Professions, and by the State Nurse Practice Act. A Licensed Practical Nurse will follow the mission, vision, I-CARE values, policies and procedures of Mohawk Valley Health System.

• Maintain a cooperative relationship among the healthcare team by effectively communicating information
• Respond to patient requests, communicates effectively to the licensed health care provider who can fulfill the patient request

Education/Experience Requirements

Required:

• Graduate of an accredited school of licensed practical nursing program

Preferred:

• Bachelors or Masters of Science in Nursing from accredited nursing program

Licensure/Certification Requirements

Required:

• Current New York State licensure as a Licensed Practical Nurse;
• Basic Life Support Certification - Must receive BLS certification within 6 weeks of hire date
