Job summary

Country Meadows in York, PA is seeking an experienced LPN to provide resident-centered care in a homelike setting. Pay starts at $32.50–$34.50 per hour, based on shift preference, with opportunities to earn more through experience.

You will use your clinical assessment skills beyond the med cart and help build care plans with physicians and families. We offer competitive benefits including medical, dental and vision, a 401(k) with company match, paid time off, and tuition reimbursement.

Qualifications

• LPN license in the state of employment.
• Minimum of one-year experience in assisted living or health care setting.
• Leadership skills and ability to form relationships with residents, families and coworkers.

Responsibilities

• Form bonds with residents, families and physicians to create care plans using critical thinking and a big-picture outlook.
• Coach coworkers to monitor and support resident needs with urgency and responsiveness.
• Round with physicians, oversee orders and monitor medications to ensure quality care around the clock.
• Educate residents and families about health and wellness issues relevant to the care plan.
• Serve as and share responsibilities of Manager on Duty.
