Job summary

Prosper Life Care, Inc. in Wickliffe is seeking a motivated LPN for our assisted living community. The role involves collaborating with an interdisciplinary team to assess needs within an Individual Service Plan (ISP) and coordinate services accordingly.

You will work with the Resident Services Director and Caregivers to deliver optimal resident care, supervise staff, and perform various nursing skills as required. Strong coordination and documentation are essential for success.

Qualifications

• Must hold valid LPN license in good standing.
• Experience in assisted living or long-term care preferred.
• Ability to assess residents and determine needs within an ISP.

Responsibilities

• Coordinate care with Interdisciplinary team and families.
• Supervise caregivers and provide staff guidance as needed.
• Develop and coordinate community educational programs with the Resident Services Director.
• Supervise resident care and initiate corrective action as necessary.
• Review admitted residents and implement ISP with the team.
• Identify safety hazards and take corrective action.
• Complete reports accurately and adhere to schedules.
