Position Summary:Under the direction of a provider or registered nurse:Provide nursing care in a community health ambulatory care setting including health promotion, health maintenance, disease management, coordination of care, and maintaining continuity of care.Provide telephone support to patients calling in to NFP of clinical needs and appropriate course of action per protocol.Provide nurse visits for clinical support and care.Essential duties and responsibilities which must be performed in order to carry out the position summarized above:(The following description is a general representation of the key duties and responsibilities of this position. Other duties may be assigned, as required.)Obtain baseline data on all patients including histories, vital signs, lab tests, and health promotion as indicated by the patient presentation.Collect, document, and report on patient data to RN or Provider to assist with accurate patient assessments and contribute to the planning, implementation, and overall evaluation of patient care.Assure the timely and accurate documentation of patient care observations and activities.Provide ongoing support to patients regarding specific health or psychosocial problems. Utilize established guidelines and educational materials.Evaluate patient knowledge regarding health, illness, and self-care practices; promote positive lifestyle and health habits.Work with clinical staff to develop and teach classes on health topics to patients and community residents as requested by the clinic.Demonstrated proficiency in utilizing the EMR system for documenting patient care, reviewing the record for health maintenance needs, entering orders, medication refill requirements, scheduling appointments, and processing the in-basket folders and messages.Collaborate with public health and social service agencies regarding patient health issues.Serve as a member of a Patient Center Medical Home team (PCMH); confer with team members and appropriate community resources.Provide comprehensive telephone support by evaluating incoming patient concerns, determining appropriate course of action within scope of license and NFP protocols, and collaborating with RN and Providers to support positive clinical outcomes.Support targeted care management for high-risk patient populations in alignment with strategic initiatives to improve long-term health outcomes.Maintain goals to align with quality metrics.Complete organizational trainings and assigned education courses as they relate to nursing role.Other duties as assigned.Qualifications:Specific knowledge, training or skills required to perform the duties of this position.
