Job summary

The Center at Blue Hills in Massachusetts is seeking a Licensed Practical Nurse (LPN) to join our team across all shifts, including days, evenings, and nights. You will provide compassionate, hands-on patient care to residents in a supportive, collaborative environment.

As an LPN, you’ll administer medications, monitor vital signs, and assist with activities of daily living under the supervision of a registered nurse.

Qualifications

• Current LPN license in Massachusetts (MA).
• High school diploma or equivalent required.
• Experience in long-term care preferred but not required.

Responsibilities

• Provide direct patient care under supervision of a registered nurse, including administering medications, vital signs, and assisting with activities of daily living
• Monitor and report changes in patients' conditions to registered nurses and other healthcare team members
• Develop and implement individualized care plans to meet the unique needs of each patient
• Collaborate with healthcare team members to ensure effective communication and coordination of patient care
• Maintain accurate and detailed records of patient care and treatment
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
