Job summary

Qualifications

• Current LPN license in Massachusetts.
• 1+ year of experience in long-term or acute care.
• BLS certification is required.
• IV therapy certification or willingness to obtain.
• Ability to work days, evenings, and weekends.

Responsibilities

• Provide high-quality, patient-centered care to residents, including medications, wound care, and vital signs.
• Collaborate with the interdisciplinary team to develop and implement individualized care plans.
• Monitor patient progress and report changes to the healthcare team.
• Participate in quality improvement initiatives and care-model development.
• Maintain accurate records of patient care and treatment.
• Educate patients and families about various aspects of care.
