Job summary

ARIA of Waukesha is seeking an experienced Licensed Practical Nurse (LPN) to provide quality nursing care to residents and supervise CNAs. The role requires a current Wisconsin LPN license and teamwork to coordinate care with other disciplines.

The LPN will administer medications, transcribe orders, document resident information, and participate in care planning and ongoing education to ensure safety and excellent care outcomes.

Qualifications

• Must have current and active Wisconsin LPN license.
• Coordinate resident care with other disciplines per regulations.
• Provide quality nursing care and supervise CNAs as assigned.

Responsibilities

• Prepare and administer medication as ordered by the physician.
• Review medication orders for completeness and accuracy.
• Transcribe physician orders accurately.
• Document resident information and notify physician/family as needed.
• Assist in Admission/Discharge processes.
• Complete accident/incident reports and follow up investigations.
• Participate in care planning conferences and updates.
• Build rapport with residents, families and staff to ensure needs are met.
• Engage in continuing education to maintain licensure and skills.

Must have current and active license to practice as a Licensed Practical Nurse (LPN) in Wisconsin.
Licensed Practical Nurse (LPN) Job Overview:
The primary purpose of the Licensed Practical Nurse (LPN) is to provide quality nursing care to our residents and to supervise the day-to-day nursing functions performed by the Certified Nursing Assistants. The Licensed Practical Nurse (LPN) is also responsible to coordinate all aspects of a resident’s care with other disciplines in the facility. Job duties are in accordance with the current Federal and State regulations that govern the facility.
Licensed Practical Nurse (LPN) Job Duties:

• Prepare and administer medication as ordered by the physician.
• Review medication orders for completeness of information and accuracy.
• Transcribe physician orders.
• Document all resident information as necessary, notifying physician and family as needed.
