Job summary

The Neighborhood of Somerset is seeking a Licensed Practical Nurse (LPN) to join our care team. The LPN reports to the Wellness Director and supports resident health, medication administration, and wellness initiatives.

Shift is Mon-Fri with every other weekend required, emphasizing leadership in care delivery. Responsibilities include coordinating PSP assessments, documenting clinical changes, and assisting with monthly wellness visits.

Qualifications

• Must be a Licensed Practical Nurse or Registered Nurse.
• Minimum of one year of experience with elders or disabled individuals.
• Demonstrated Leadership capabilities.
• Knowledge/experience in Assisted Living and Dementia care preferred.
• Ability to work weekends, evenings and flexible hours.

Responsibilities

• Identify residents' ongoing needs through the PSP process and reflect them in personalized care plans.
• Ensure proper follow-through and documentation for changes in clinical care needs.
• Assist the Wellness Director with monthly wellness visits and PSP updates.
• Inform Wellness/Executive Directors of changes in medications or services that may affect care tier or move-out.
• Assist with obtaining weights and vital signs monthly for wellness updates.
• Provide direct resident care as needed.

The Neighborhood of Somerset is seeking a LPN to join their team! The LPN (Nurse MedTech) reports directly to the Wellness Director.

Shift Details

Mon-Fri ; Every other weekend required

PURPOSE

The Wellness Nurse/Nurse Med Tech is responsible for monitoring the health and wellbeing of our residents and provides leadership support and assistance with resident care and services. Responsibilities include but are not limited to medication care administration, communication with resident support services and families and maintaining clinical quality assurance while leading, demonstrating and supporting all the elements of wellness which are core components of Phoenix programs and services.

Principle Duties And Responsibilities

LPN - Licensed Practical Nurse

Resident Care

• Identifies on-going needs and services of residents through the assessment / Personalized Service Plan (PSP) process
• Ensures proper follow through and documentation for residents with a change in clinical care needs
• Assist the Wellness Director with Completing the clinical sections of monthly wellness visits and PSP to accurately reflect the resident monthly
• Informs the Wellness Director and/or Executive Director of any changes in medication or service level that may result in a higher tier level or potential move-out
• Assist with obtaining weights and vital signs monthly for each resident prior to completing monthly wellness health updates
• Assist with direct resident care as needed

Education/Experience/Licensure- Certification

LPN - Licensed Practical Nurse

• Must be a Licensed Practical Nurse or Registered Nurse
• A minimum of one year of experience working with elders or disabled individuals, preferred
• Must have demonstrated Leadership capabilities
• Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred
• Ability to work weekends, evenings and flexible hours, available to our customers at peak service delivery days and times
