Job summary

Oaks Integrated Care in New Jersey is seeking a licensed Practical Nurse to provide medication monitoring and education, participate in CSP planning, and train staff and families. The role involves on-call emergency support, coordination of patient assessments, and collaboration with healthcare teams in a residential MH setting.

The position requires an NJ LPN license, nursing experience, and a valid driver’s license.

Qualifications

• Must hold a valid NJ Licensed Practical Nurse license.
• Familiarity with medication and uses, and ability to educate staff.
• Valid driver’s license and pre-employment drug screen requirement.

Responsibilities

• Provide medication monitoring
• Provide emergency evaluation as needed
• Participate in CSP development and revision
• Provide staff, consumer, and family training
• Administer intravenous and non intravenous medications
• Monitor accuracy of physician orders
• Coordinate ongoing assessments
• Assist in crisis interventions
• Face to face health care monitoring
• Collaborate with healthcare providers
• Be available for on-call emergencies 24/7
• Complete supporting documentation
• Flex to work across MH residential programs

• Pay or shift range: $26 USD to $28 USD
  The estimated range is the budgeted amount for this position. Final offers are based on various factors, including skill set, experience, location, qualifications and other job-related reasons.

The qualified candidate should: have familiarity with medication and uses; ability to communicate medication education to staff; ability to teach coping skills and gain and maintain trust; understand negative impact of mental illness and developmental disabilities; and have the ability to be an advocate for consumers.

Responsibilities:

• Provide medication monitoring
• Provide emergency evaluation as needed
• Participate in the development and revision of Comprehensive Service Plans (CSP)
• Provide training to staff, consumers, and family members
• Administer intravenous and non-intravenous medications
• Monitor for accuracy of and confirm physician’s orders
• Coordinate and provide on-going assessments
• Assist in crisis intervention situations as needed
• Face to face health care monitoring
• Collaborate with healthcare providers
• Flexibility to be pulled to other MH residential programs
• Complete supporting documentation for all of the above duties
• Provide on call support 24 hours for emergencies
• Other duties as assigned.

Qualifications:

NJ Licensed Practical Nurse (LPN) License;

Nursing experience required;

• Valid driver’s license.
