Job summary

Tennessee Quality Care is hiring a Full-Time Licensed Practical Nurse (LPN) to provide one-on-one Home Health care to patients throughout Shelby County, TN and the Memphis area. You’ll work a Monday–Friday schedule with competitive hourly pay and mileage reimbursement, plus PTO, holidays, and benefits.

Qualifications

• Current LPN/LVN license in Tennessee.
• Graduate of an accredited practical or vocational nursing program.
• Current CPR certification.
• Reliable transportation and driving requirements.
• Valid driver's license and automobile insurance.
• 3+ years of nursing experience preferred.
• Home Health, Medical‑Surgical, Community Health, or similar nursing experience preferred.

Responsibilities

• Provide direct patient care in patients’ homes according to the plan of care.
• Complete skilled nursing visits in patients’ homes.
• Administer treatments and medications as ordered.
• Monitor patients and report changes to the RN.
• Educate patients and families on medications, treatments, and self-care.
• Complete accurate and timely clinical documentation.
• Follow infection‑control and safety procedures.
• Collaborate with the RN, physicians, therapists, and care team.
• Support patients in managing chronic conditions and maintaining independence at home.

What You’ll Do

As a Home Health LPN, you’ll provide skilled nursing care directly in patients’ homes while working closely with the Registered Nurse and interdisciplinary care team.

Responsibilities include:
