Addus HomeCare in Lebanon, TN is seeking a Full-Time Licensed Practical Nurse to provide direct patient care in home health settings across Lewis, Perry, and Hickman Counties. You will implement plans of care and document outcomes while maintaining patient comfort and safety.

The role offers competitive hourly pay, a flexible weekday schedule, mileage reimbursement, and comprehensive benefits. Must be an LPN in TN with CPR, driving privileges, and 3 years nursing experience.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience; community/home health or medical/surgical experience preferred.
• Currently licensed as an LPN/LVN in Tennessee.
• Possess and maintain valid CPR certification.
• Must have reliable transportation, valid driver’s license and auto insurance.

Responsibilities

• Provides direct patient care as defined in the state Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation in accordance with the plan of care.
• Educates patient and family/caregiver on self-care and diagnosis-related information.
• Coordinates home health services and reports needs to the RN/Clinical Supervisor.
• Uses and maintains equipment and supplies with aseptic technique.
• Participates in personal and professional growth and development.
• Performs other duties as assigned by the RN.
• Delivers health care instructions per assessment and plan of care.

What You’ll Do:

• Provides direct patient care as defined in the state Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
• Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
• Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique.
• Participates in personal and professional growth and development.
• Performs other duties as assigned by the registered nurse.
• Provides health care instructions to the patient as appropriate per assessment and plan of care.
