The Licensed Practical Nurse (LPN) at I Am Boundless, Inc. provides nursing services to residents of our Intermediate Care Facility in Independence, OH.

Qualifications
- Graduate of approved school of practical nursing.
- LPN license required.
- Must communicate in English (written and spoken).

The Licensed Practical Nurse provides nursing services to individuals residing in our ICFs (Intermediate Care Facility). The LPN reports to a Registered Nurse and is responsible
for working within the scope of their license to ensure that the physical health needs of the clients are met and maintained on an individualized basis in accordance with policies and proceduresandcurrentfederal,state,andlocalstandards,guidelines,andregulations. LPNwill coordinatehealthneedswith membersoftheclient's teamaswellasprovidenecessarytraining to team members. LPN will also be responsible for delegating nursing tasks to certified direct service providers.

- Acting on direction of a licensed physician, licensed dentist or at the direction of a registered nurse administers controlled and non-controlled medication (i.e., oral, rectal, and injection); maintains medication accountability records; maintains medication equipment and supplies; performs skilled nursing procedures (e.g., applies and changes dressings,catharizes);assistsnurseinmedicalprocedures;requests,securesandfollows up on medical information of residents.
- Providesgeneralnursingcaretoresidents;maintains first-aidsuppliesandprovidesfirst- aid treatment as necessary; screens for communicable and nuisance diseases.
- Observesanddocumentsconditionsandresponsesto,andprogressof,nursingcare treatment and medications; reports unusual developments to the supervisor.
- Supervises serving of diets assuring that individuals unable to feed themselves are properly fed and that special eating utensils are available as needed; notes and records special problems related to eating and fluid intake; reports unusual problems to the DietitianandPhysician;providesinstructionand trainingtoresidentsinpersonalhealth- related matters (e.g., nutrition, grooming, hygiene, safety)
- Preparesandmaintainswrittenrecordsandreportsasrequired;ensurescompliancewith all reporting standards; accepts on-call rotation
- Participates as member of the interdisciplinary team working with other professional staff assessing, developing, implementing, evaluating and modifying Individual Habilitation Plans.
- Providesconsultationservicestostaff,residents,parentsandsignificantothersregarding health and safety related issues; promotes consistency of programming outside of residential environment; operates county vehicles.
- Participatesininfectioncontrol,pharmacyandtherapeutics,committeemeetingsand others as necessary
- Performsotherrelateddutiesasassigned.
- Mustbeatleast18 years of age
- Graduateofanapprovedschoolofpracticalnursing
- LPNLicense-StateBoardofNursing
- MustbeabletocommunicateinEnglish,includingbothspokenandinwriting.
- Ability to obtain and maintain First Aid, CPR - adult, child, and infant; delegated nursing. (training will be provided, if not already certified)
- Valid Ohio Driver’s License with Ohio Bureau of Motor Vehicles
- Proofofautoinsurance.
- OneyearexperienceworkingasaLPNwithintellectuallyordevelopmentallydisabled individuals preferred.
- Mustbedetailorientedandhavecompetentliteracyandwriting,documentation, communications and interpersonal abilities.
- Mustbeabletousetoolsandequipmentasrequired –includingpushing/pulling wheelchairs, lifting consumers with hoyer lifts
