Job summary

Symbii Home Health and Hospice is seeking a Licensed Practical Nurse (LPN) to join their team in Idaho Falls, Idaho. Licensed Practical Nurse (LPN)

Symbii Home Health and Hospice is seeking a Licensed Practical Nurse to join our amazing team!

What will you do as an LPN at Symbii?

• Provide direct patient care.
• Implement plan of care initiated by the registered nurse.
• Document accurately and timely in accordance with the plan of care.
• Collect assessment data and perform observations to report to and collaborate with RN(s) and physician(s).
• Coordinate home health services, reporting identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietitian) to the registered nurse and/or Assistant Director of Nursing or Director of Nursing.
• Participate in personal and professional growth and development.
• Perform other duties as assigned by the registered nurse.

Job Requirements

• Must possess a valid LPN license in the State of Idaho.
• Must hold a current CPR certification.
• Must hold a valid driver’s license and current auto insurance.
