vTech Solution is seeking a Licensed Practical Nurse (LPN) for a Rehabilitation and Extended Care Facility in Helena, MT. The LPN will provide compassionate nursing care, collaborate with the interdisciplinary team, and manage clinical and administrative duties in a dynamic healthcare setting.

The ideal candidate holds an LPN license (New York) and has at least 2 years of experience, with strong communication and teamwork skills, capable of lifting 45 pounds and working a night shift from 7:00

Qualifications

• Active and unencumbered New York State LPN license is required.
• At least 2 years of LPN experience.
• Strong communication and collaboration skills.
• Ability to work efficiently in a fast-paced environment.
• Physical ability to lift 45 pounds or more.

Responsibilities

• Provide compassionate care and treatment to residents with a customer service attitude.
• Collaborate with the interdisciplinary team including Physicians, RNs, Unit Managers, CNAs, and Ward Clerks.
• Evaluate and observe changes in resident conditions.
• Communicate promptly with RNs and Physicians regarding changes in resident status.
• Delegate assignments and supervise support staff to ensure appropriate care delivery.
• Order medications and supplies, prepare equipment, and assist with wound care as needed.
• Maintain a clean and hygienic environment for residents.
• Schedule resident appointments and notify families about appointments and transportation arrangements.
• Input orders into computer systems, notify pharmacy, and maintain accurate documentation.
• Attend meetings and participate in care meetings and discharge planning.
• Ensure compliance with policies related to controlled substances and documentation.
• Report incidents of abuse, neglect, or exploitation promptly.

The role requires sound nursing judgment, effective communication, and the ability to manage clinical and administrative duties in a dynamic healthcare environment.

Responsibilities:

• Provide compassionate care and treatment to residents with a customer service attitude.
• Collaborate effectively with the interdisciplinary team including Physicians, RNs, Unit Managers, CNAs, and Ward Clerks.

• Ensure compliance with policies related to controlled substances and documentation.
• Report building emergencies and incidents of suspected abuse, neglect, mistreatment, or exploitation promptly.

Required Skills & Certifications:

• High School diploma or equivalent.
• Completion of Licensed Practical Nurse course of study.
• Active and unencumbered New York State LPN license.
• At least 2 years of LPN experience.
• Strong communication and collaboration skills.
• Ability to work efficiently in a fast-paced, dynamic environment.
• Physical ability to lift 45 pounds or more, perform repeated bending, kneeling, and hand-eye coordination tasks.

Preferred Skills & Certifications:

• BLS/CPR certification preferred.

Special Considerations:

• Must maintain compliance with controlled substance policies and accurate documentation.
• Responsible for reporting any suspected abuse, neglect, or exploitation immediately.
• Work environment includes Rehabilitation and Extended Care Facility with patient care and administrative duties.

Scheduling:

• Nights shift from 7:00 PM to 7:30 AM.
• Every other weekend and holiday work as per unit needs.
