Job summary

The Gardens Health and Rehab in Daytona Beach, FL is seeking an LPN to join our skilled nursing team. You will provide nursing care to residents as prescribed by physicians and in accordance with standards of practice, with emphasis on documentation and quality care.

This full-time role offers a sign-on bonus and a collaborative work environment focused on resident well-being, professional development, and compassionate service. Florida licensure and CPR certification are required.

Responsibilities

• Assess and document resident progress in the medical record.
• Monitor turning and positioning of residents.

• Communicate pertinent information about resident condition to charge nurses and Director of Nursing.
• Maintain clean, organized treatment cart and restock supplies.
• Follow Infection Control policies and procedures.
• Administer medications and treatments as ordered by the physician.
• Document pressure sore status weekly.
• Monitor inventory of medications and supplies.
• Educate residents and families on care as needed.
• Participate in Quality Improvement activities.

$4,000 Sign on Bonus for Full-Time AND $2,000 Sign on Bonus for Part-Time!

Summary of the Licensed Practical Nurse (LPN):

Provide nursing care to residents as prescribed by the physician and in accordance with standards of nursing practice and regulations and as directed by the Director of Nursing.

Job Requirements of Licensed Practical Nurse (LPN):

• Licensed Practical Nurse current state license in Florida.
• At least one (1) year of Skilled Nursing experience preferred.
• Must have a current/active CPR certification.
• Excellent technical, assessment, and documentation skills.
• Effective verbal and written English communication skills.
• Highest level of professionalism with the ability to maintain confidentiality.
• Ability to communicate at all levels of organization and work well within a team environment in support of company objectives.
• Customer service oriented with the ability to work well under pressure.
• Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity.
• Strong analytical and problem solving skills.
• Ability to work with minimal supervision, take initiative and make independent decisions.

Essential Duties & Responsibilities of Licensed Practical Nurse (LPN):

• Assess and document resident progress in the medical record.
• Monitor turning and positioning of residents.
• Obtain changes in physician’s orders as indicated by resident’s progress or lack of progress
• Communicate pertinent information about resident condition and progress to charge nurses, supervisors and Director of Nursing, as indicated.
• Maintain clean, orderly treatment cart and restock treatment supplies.
• Follow Infection Control policies and procedures.
• Administer medications and treatments accurately as ordered by the physician and according to all facility policies and procedures.
• Document pressure sore status weekly or more often as condition changes.
• Assist in monitoring the inventory of medications, medical supplies and equipment to be sure adequate supplies of skin care products are on hand to meet the needs of residents.
• Participate in educational programs for residents and their family members relative to skin care when a resident is being discharged to home.
• Participate in Quality Improvement activities, as assigned.
