Job summary

Rocky Mountain Health Care Services is seeking an LPN to provide skilled nursing care within an interdisciplinary team for participants, including education to families and caregivers. The role supports care plans and fosters independence for patients in the Colorado region.

The LPN will perform phlebotomy, wound care, and med administration, while coordinating with physicians and facilities. Strong communication, reliability, and EMR proficiency are essential to success in this position.

Qualifications

  • Graduate of an accredited LPN program and licensed as a professional in Colorado.
  • Minimum 1 year of health care experience with frail or elderly population preferred.
  • Ability to perform phlebotomy and handle medical equipment.
  • Strong written and verbal communication skills and ability to interact with internal and external customers.

Responsibilities

  • Provide skilled nursing care procedures and administer medications per licensure.
  • Educate participants and caregivers on medications, inhalers, and testing.
  • Document assessments and care in the EMR; coordinate with IDT and family.
  • Perform phlebotomy and lab specimen collection as assigned; handle specimens with proper precautions.
  • Coordinate with facilities and pharmacy; ensure follow-up after ER visits or falls.

POSITION SUMMARY: LPNs are responsible for providing skilled nursing care to participants. This position is responsible for working directly with the Interdisciplinary Team (IDT), participant, and his or her representative to achieve the goals of the care plan. Identifies and labels specimens appropriately. Handles potentially infectious specimens with appropriate biohazard precautions, and practices universal precautions.
