Job summary

MindCare Solutions in Pittsburgh, PA seeks an experienced LPC/LCSW to deliver direct counseling, care coordination, and telehealth services. You will collaborate with physicians and the care team to integrate behavioral health into patient care and develop personalized treatment plans.

The role is full-time (Mon–Fri days) with travel up to 50% and a focus on evidence-based care, screening tools, documentation, and program development to meet patient and community needs.

Qualifications

  • Master’s degree in counseling, Social Work, or related field.
  • Active and unrestricted PA LPC or LCSW license.
  • Minimum of 1 year of clinical experience in behavioral health.
  • Strong telehealth proficiency and ability to adapt to new technologies.
  • Excellent communication and collaboration skills.
  • Ability to travel up to 50%.

Responsibilities

  • Provide direct counseling and therapeutic interventions to patients, utilizing evidence-based approaches.
  • Develop personalized care plans addressing psychological and emotional challenges.
  • Collaborate with primary care providers, psychiatrists, and other healthcare team members to integrate behavioral health services into patient care.
  • Act as a resource for medical staff, ensuring alignment with evidence-based behavioral health practices.
  • Deliver telehealth services to patients, ensuring accessibility and high-quality care in virtual settings.
  • Utilize telehealth technology to expand access to care and reduce barriers for patients.
  • Administer, score, and interpret validated behavioral health screening tools to identify patient needs.
  • Document screening outcomes and provide feedback to patients and the care team.
  • Develop action plans based on screening results and ensure follow-ups.
  • Act as a patient advocate to ensure timely access to appropriate behavioral health services.
  • Educate patients and families about behavioral health conditions coping strategies, treatment plans, and available resources.
  • Provide case management services to ensure continuity of care, including referrals to community resources and specialized services.
  • Monitor and document patient progress, modifying care plans as needed.
  • Assist in the development and implementation of behavioral health programs to meet patient and community needs.
  • Monitor program outcomes and recommend improvements.
  • Provide training to healthcare staff on behavioral health topics, tools, and best practices.
  • Serve as a resource for patients, families, and providers regarding behavioral health care.

LPC/LCSW

The LPC/LCSW supports the delivery of high-quality behavioral health services through counseling, care coordination, and collaboration. This role includes overseeing behavioral health assessments, providing telehealth support, and implementing behavioral health screening tools to optimize patient care outcomes. The role is instrumental in fostering interdisciplinary collaboration and ensuring seamless integration of behavioral health into comprehensive care plans. Key responsibilities include collaborating closely with clinical teams to share clinical expertise and program coordination to provide patient-centered, evidence-based care while ensuring compliance with organizational and regulatory standards.

Essential Duties And Responsibilities

  • Knowledge of behavioral health screening tools and evidence-based therapeutic interventions.

  • Maintain clinical competency and compliance with all applicable regulations.
