Job summary

Vital HealthCare Solutions is expanding its Psychology Department to include Licensed Clinical Social Workers (LCSWs) and Licensed Professional Counselors (LPCs) who will provide individual psychotherapy to residents of skilled nursing facilities and assisted living communities in the Northeast.

You will conduct evaluations, develop tailored plans, and work closely with medical teams in a collaborative, supportive setting.

Qualifications

• Active LCSW or LPC license (required).
• Experience in geriatric mental health, long-term care, or behavioral health (preferred).
• Compassionate, collaborative clinicians who thrive in interdisciplinary settings.

Responsibilities

• Deliver individual psychotherapy and counseling using evidence-based practices.
• Conduct comprehensive psychological evaluations and diagnostic assessments.
• Develop and implement tailored treatment plans based on patient needs.
• Collaborate with Psychology and Psychiatry teams and facility-based medical staff for integrated care.
• Work with a diverse population presenting a wide range of emotional and behavioral health needs.

What You’ll Do:
