We are seeking a compassionate skilled Nurse (RN or LPN) in Chimayo, NM to provide patient care in accordance with a physician-established care plan.
This is a 1099 contract opportunity, with care provided at the patient’s residence.
Patient needs 4 hours of care a month. Patient would like 2 visits of 2 hours on Tuesday, Wednesday or Thursdays.
Once patients are accepted into the program, there is no expiration for this benefit, allowing our caregivers to build lasting relationships with their patients for years to come.
In this role, you will advocate for the patients and work one-on-one to deliver the care they need. You will collaborate with CNAs, HHAs, Case Managers, and the patient’s physicians.
