CVS Pharmacy is seeking a Certified Peer Support Specialist for field-based work and requires residence in Louisiana (SE region). This full-time role involves visiting members in the community, coaching toward recovery goals, and collaborating with care teams.

Travel up to 50% to office and community locations is expected. Promising candidates will leverage lived experience to guide care coordination, document non-clinical notes, and support member engagement with resources.

Qualifications

• Must reside in Louisiana, SE region (Baton Rouge, Northshore, New Orleans).
• Ability to travel 50% with personal vehicle; CVS reimburses mileage.
• 1-2 years’ experience in community/outpatient mental health or substance use setting.
• 2-3 years knowledge of community resources and multi-system outreach.
• Experience educating health care providers on cultural, ethnic, or religious preferences.
• Proficiency with MS Office and EHR systems.

Responsabilités

• Attend case rounds and identify members who may benefit from peer services.
• Educate care teams on how peer support helps recovery and care plans.
• Collaborate with healthcare professionals on recovery principles and social determinants.
• Assist members in identifying socio-economic issues and developing person-centered plans.
• Meet with members for strengths-based, person-centered goal setting.
