Position Overview

Registered Nurse – Community Living Center (CLC)

Compensation: 50.00/hr plus $5.09 for Health and Wellness

We are seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality care in a Community Living Center (CLC) setting. This role focuses on short-term rehabilitation, skilled nursing care, and long-term support for Veterans transitioning from acute care to home or community environments.

Responsibilities
  • Conduct comprehensive patient assessments and develop individualized care plans
  • Administer medications, IV therapy, tube feedings, and wound care
  • Provide tracheostomy care, catheter management, and chronic disease support
  • Assist patients with activities of daily living (ADLs)
  • Collaborate with interdisciplinary teams to support recovery and discharge planning
  • Document care accurately in the electronic health record (EHR)
  • Respond to medical emergencies and changes in patient condition
Work Environment
  • Post-acute rehab, dementia care, hospice, and skilled nursing units
  • Patient population includes rehabilitation, respiratory, cognitive impairment, and end-of-life care
Minimum Qualifications
  • Graduate of an accredited nursing program (ACEN or CCNE)
  • Active RN license (Ohio or compact state)
  • Current BLS certification (ACLS required for ICU/ED)
  • Minimum 2 years of recent clinical experience
  • Strong English communication and documentation skills
Additional Requirements
  • Ability to work 12-hour shifts, including nights, weekends, and holidays
  • Completion of VA-required training (safety, HIPAA, EHR, etc.)
  • Compliance with credentialing and privileging standards
  • Ability to float between units as needed
