Job summary

Sweetwater Care in Porterville, CA is seeking an LVN/LPN to provide nursing care within scope of practice, following physician orders and center policies. The role supports patient needs in a licensed long-term care setting and requires charting and coordination with medical staff.

The ideal candidate has long-term care experience, holds an active LVN/LPN license in California, and is BLS/CPR certified. Experience in nursing management is a plus but not required.

Qualifications

• Experience in long-term care settings.
• Active LVN/LPN license in California.
• Current BLS/CPR certification or willingness to obtain.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including nutritional problems, infections, skin conditions, weight loss, or other noticeable changes.
• Utilizes the Quality Life Manual for patient identification, placement, and monitoring in appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
