MedCap Health is seeking a Long Term Care Doctor - Post-Acute Care Physician to join our physician-led team. We offer a flexible schedule—full-time, part-time, or customized hours—to fit your life while delivering hospital-level care in skilled nursing facilities.

You’ll lead care teams, manage complex post-acute cases, and collaborate with NPs and specialists to ensure continuity, quality, and compassionate patient care in a dynamic post-acute setting.

Qualifications

• Board-certified or board-eligible in IM, CCU, FP, or geriatrics.
• Experience leading care teams in post-acute or hospitalist settings.
• Ability to coordinate care with NPs and specialists.

YOUR CONTRIBUTIONS MATTER

As our Long Term Care Doctor - Post-Acute Care Physician, you'll lead care teams in skilled nursing facilities, managing complex cases discharged early from hospitals. You'll collaborate with Nurse Practitioners and specialists to deliver hospital-level care in a long-term setting. Your leadership ensures continuity, quality, and compassion in every patient interaction.

WHAT'S REQUIRED?

• Board Certification or Board Eligible internal medicine, critical care, family practice, or geriatrics

Previous experience in leadership positions or as a hospitalist is strongly preferred.
