An exciting opportunity for an Advanced Practice Provider (APP) to practice autonomously while collaborating closely with private practice, critical care and pulmonary-trained physicians. The Advanced Practitioner will collaborate with all members of the multidisciplinary care team to deliver high-quality care across multiple settings, including long-term care and skilled nursing facilities (LTC/SNF), private practice office, medical centers and telehealth. This role offers a high degree of professional autonomy, supported by physician oversight and availability.

Core responsibilities may include:

• Multidisciplinary rounding;
• Performing history and physicals/daily progress note documentation;
• Ventilator management;
• Clinical response to change in patient condition;
• Family discussions and meetings;
• Bedside procedures to include central line/hemodialysis catheter insertion and arterial lines.
