• No 25+ points in the last 3 years.
• No DOT Preventable accidents in the last 5 years; no more than 2 preventable in 3 years.
• No more than 6 DOT employers in the last year.
• Must live within 50 miles of the Portage Terminal 49024 and have transportation to the facility.
