MyCare Health Center seeks a Licensed Master Social Worker (LMSW) or Licensed Professional Counselor (LPC) to provide case management and therapeutic services in a split 0.5 FTE role focused on justice-involved individuals and community patients.

The position supports assessments, treatment planning, counseling, and coordination of care across primary care, behavioral health, substance use disorder services, and social resources.

Formación

• Master’s degree in Social Work required.
• Current Michigan licensure (LMSW or LPC) required.
• Minimum of 2 years counseling and/or case management experience (5+ years post-licensure preferred).
• Experience with assessments, treatment planning, evidence-based therapy, and documentation.
• CADC/CAADC or Development Plan preferred.
• BLS Certified.

Responsabilidades

• Conduct comprehensive assessments of medical, behavioral, and social needs, particularly for individuals transitioning from incarceration.
• Develop, implement, and monitor individualized care plans in collaboration with the patient and care team.
• Facilitate linkage to primary care, behavioral health, SUD services, and community resources.
• Provide education, counseling, and ongoing support to patients and families regarding health conditions and recovery goals.
• Coordinate care transitions and advocate to reduce barriers to accessing care.
• Participate in an integrated team approach to patient care; team includes physicians, RNs, peers, clinicians.
• Maintain documentation in accordance with Medicaid, state, and federal requirements.
• Collaborate with Medicaid health plans, corrections systems, and community partners for justice-involved individuals.
• Respond to crises and provide appropriate interventions within scope of practice.
